Description & Services
Service Details - Offers a FREE online, evidence-based, health education program that provides information, tools and practical skills on managing long-term health conditions 
- Increases the confidence and motivation people need to better manage the challenges of living with a chronic health condition 
- Participants log-on to the program website at their convenience, for 2-3 hours per week, in a safe and anonymous environment to give and receive support from others who are experiencing similar health issues 
- Programs are highly interactive and have 20-25 persons per group 
- Participants are provided with an accompanying resource text and resource materials at no cost  
- Family members and caretakers are also welcome to participate
Eligibility Ages: 19 year(s) and up 
- Workshops are offered on an ongoing basis throughout the year 
- Workshop assignment is conducted based on a first come-first served basis on the website 
- Plan to commit to all six sessions
