Description & Services

Service Details -Our programs are offered online, virtually, and over the phone, thus people can participate from the comfort of their own homes.  
-Our programs are offered in Chronic Pain, Chronic Conditions, Diabetes and Cancer: Thriving & Surviving  
-We have 5 Different Format offerings of our Programs: Tool Kit for Active Living plus Calls; Tool Kit for Active Living, virtual 6 week Self-Management Workshops, Better Choices Better Health (online workshop) and our Telephone Health Coach Program  
- FREE scientific evidence-based, health education programs delivered by trained program leaders that provide information, tools and practical skills on managing chronic health conditions 
- Increases the confidence and motivation people need to better manage the challenges of living with a chronic health condition, chronic pain, diabetes 
- Participants share in a safe and confidential environment and give and receive support from others who are experiencing similar health issues 
- Programs are highly interactive, fun and not a 'sit-and-listen' lecture format 
 
1) Tool Kit for Active Living plus Calls: trained leaders meet with groups of 6 people for a 45 minute session on Zoom, once each week for six consecutive weeks and participants receive our Tool Kit for Active Living kit which includes a Living a Healthy Life with Chronic Conditions or Chronic Pain Resource Book, 2 CD's (relaxation for Mind & Body and an exercise CD), a self-test to help decide how to use the materials and tip sheets on the most important self-management tools.  
 
2) Tool Kit for Active Living: participants receive the tool kit: Living a Healthy Life with Chronic Conditions or Chronic Pain Resource Book, 2 CD's (relaxation for Mind & Body and an exercise CD), a self-test to help decide how to use the materials and tip sheets on the most important self-management tools; and work through the program independently.  
 
3) Better Choices, Better Health: This asynchronous web-based, 6-week program uses content from the Chronic Conditions Self-Management Program and focuses on decision making, problem-solving, and action -planning. New lessons are presented each week, and sessions are highly participative through internal messaging and online discussion boards. There are no requirements that participants log in at the same time. 
 
4) Telephone Health Coach program: one to one telephone-based program supports people living with chronic conditions to become better self-managers. Health Coaches connect with participants by telephone, once a week for 30 minutes, for a period of three months. Health Coaches provide a dimension of support that complements and enhances professional health care. 
 
5) virtual 6 Week Self-Management workshops: participants meet with two trained peer leaders over 6 weeks. The group, maximum 12 people, meets once a week for 2.5 hours over Zoom and works through our programs to learn how to self-manage and life a healthier, happier life. These programs are offered in Chronic Pain, Chronic Conditions, Diabetes and Cancer: Thriving & Surviving. Please note that a web camera is required in order to participate.
