Description & Services

Service Details - Team of mental health professionals; generally consists of a nurse and a social worker 
- Provides mental health assessments, risk assessments, and resource navigation  
- Depending on level of risk (where there is no immediate physical risk or danger), the team can assist via phone or in-home visits 
- Does not provide ongoing case management- brief services and referrals to community supports/resource navigation only 
(***where there is immediate physical risk and/or danger, Emergency Services such as 9.1.1, should always be called***)
Hours 7 days/week, 11:30am - 9:00pm (including statutory holidays)
Eligibility Individuals experiencing a mental health crisis and/or who may be at imminent risk to harm themselves or others
How to Apply - Individuals can call inquiring for themselves  
- Family/supports can call and receive info on behalf of someone, however; 
- the service is voluntary, so the Team cannot intervene unless the individual volunteers to receive support
