| Service Details | - Works collaboratively with partners in the community - Helps parents/caregivers access relevant programs and services based on their individual needs, supports families in times of need, answers telephone-based inquiries, provides information on mental health and substance use issues and resources (online and community-based), completes an assessment and develops a plan of care with parents/caregivers.Provides education on self care/Coping Strategies and Communication Skills. Helps to access community based resources |
