Description & Services
| Service Details | Assisting older adults with navigating the housing and service systems to find, attain and maintain appropriate housing. Connecting with older adults (60+) who are housing vulnerable and providing one-on-one support for client-defined goals, including: securing and maintaining housing, accessing missing income supports, health referrals, nutritional supports, and appropriate social connections. |

| How to Apply | Call, text, or email. Information required: First and last name, date of birth, contact number, email address, mailing address, approximate income and sources of same. Please proudly let us know if you have served our country in uniform or trained for same. Note: If you have someone helping you with forms, you can list their contact info on file at your discretion. |
