| Service Details | This is an AT HOME program that helps patients being discharged from hospital to manage their Chronic Obstructive Pulmonary Disease (COPD) * provides intensive, home-based respiratory care, education, and case management to address the causes that have led to an acute care visit, including Cardio Respiratory Home assessments, COPD Flare up Plans, ongoing education and home based activity programs; Inhaled Medication Education and review, as well as Tobacco Dependency Counseling |
