| Service Details | Provides trauma-informed short-term, team-based care to higher functioning individuals experiencing a mental health illness and individuals with substance use concerns; available to individuals who have a family physician, with referrals being made by family physicians and nurse practitioners. |
| Hours | Monday-Friday, 8:30am - 4:30pm |
| How to Apply | Contact with an Interior Health Social Worker can be arranged through your Physician, Hospital Patient Care Coordinator, Residential Care Supervisor, Community Care Case Manager or other primary health care provider. Self-referrals are accepted. |
